Sex differences in the effect of Dynapenic Abdominal Obesity on the trajectories of mobility decline and lower extremity function in people over 50 years

Abstract

In isolation, Obesity and dynapenia are risk factors for mobility decline and lower extremity function in the elderly. In addition, the elderly with Dynapenic Abdominal Obesity, a combination of Abdominal Obesity and dynapenia, have a poorer trajectory of disability in basic and instrumental activities of daily living, have a higher risk of metabolic syndrome, a disorder of lipid and glucose metabolism, and a higher risk of mortality than those with only Abdominal Obesity or only dynapenia. However, there is little epidemiological evidence demonstrating the effect of Dynapenic Abdominal Obesity on the trajectories of mobility decline and lower extremity function in people over 50 years of age. In addition, no study analyzed sex differences in such trajectories. Thus, the objectives of the present proposal are: 1) to analyze the effect of Dynapenic Abdominal Obesity on the trajectory of mobility decline; 2) to analyze the effect of Dynapenic Abdominal Obesity on the trajectory of lower extremity function decline and 3) to analyze sex differences in these trajectories. To do this, the longitudinal data of the English Longitudinal Study of Ageing (ELSA), composed of 7,666 individuals older than 50 years of age, will be used, thanks to a partnership of the counselor of this proposal with the Department of Epidemiology and Public Health of University College London (UCL). The baseline of the study will be the wave of 2004/2005 and, as a follow-up, the wave data of 2008/2009 and 2012/2013 will be used, resulting in 8 years of follow-up. Mobility will be assessed by walking speed in meters / second and lower extremity function will be evaluated by the Short Physical Performance Battery (SPPB). Abdominal Obesity will be defined for waist circumference values > 102 cm for men and > 88 cm for women, and dynapenia will be considered for manual grip strength values < 26 kg for men and < 16 kg for women. Participants will be classified as (0) non-Abdominal obese/non-Dynapenic; (1) Abdominal obese/non-Dynapenic; (2) non-Abdominal obese/Dynapenic and (3) Dynapenic Abdominal obese. The effect of sex Dynapenic Abdominal Obesity on the trajectories of mobility decline and lower extremity function over 8 years of follow-up will be analyzed through Generalized Linear Mixed Models. Socioeconomic, neuropsychiatric factors, life habits, medications in use, health status, falls, hospitalization and functionality will be considered control variables. The hypothesis of the present study is that women with Dynapenic Abdominal Obesity have a greater decline in mobility and lower extremity function than men. (AU)
